The Sponsor of this promotion is JB7 LLC, 6990 West 38th Avenue, Suite 304, Wheat Ridge, CO 80033 ("Sponsor" or "Physician's Choice").

Open only to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older (nineteen (19) in AL and NE, twenty-one (21) in MS) at the time of entry. Void in Puerto Rico, all U.S. territories and possessions, and everywhere else prohibited by law.

Employees, officers, directors, and agents of Sponsor, its parent, subsidiaries, affiliates, advertising and promotional agencies, and fulfillment partners, and the immediate family members and household members of each, are not eligible.

By entering, you agree to these Official Rules, which are final and binding.

The promotion consists of consecutive weekly entry periods (each, an "Entry Period"). Each Entry Period begins at 12:00:00 a.m. Mountain Time on Monday and ends at 11:59:59 p.m. Mountain Time on the following Sunday. The promotion begins on Monday, August 24, 2026 and continues until Sponsor ends it in accordance with Section 14. Sponsor's systems are the official timekeeping device for this promotion.

There are two (2) ways to receive one (1) entry per Entry Period. Both methods carry equal weight in the drawing.

One (1) entry per outer mailing envelope. Mechanically reproduced, illegible, incomplete, or bulk-submitted entries are void. Each valid mail-in entry will be placed into the drawing for the first Entry Period that has not yet closed as of the date the entry is received by Sponsor. Mail-in entries carry the same weight as automatic subscriber entries. Sponsor is not responsible for lost, late, damaged, misdirected, or postage-due mail.

Entry limit: One (1) entry per person per Entry Period, regardless of method. Entries received in excess of this limit will be void, and an individual who is automatically entered as a subscriber and also submits a mail-in entry for the same Entry Period will receive one (1) entry only. Multiple entrants may not share the same email address or postal address, and multiple subscriptions held by the same individual or household do not result in additional entries. Use of any automated system, bot, script, or multiple accounts or identities to obtain additional entries is prohibited and will result in disqualification.

Sponsor is not responsible for lost, late, misdirected, incomplete, or unprocessed entries, or for technical failures of any kind.

For each Entry Period, one (1) potential winner will be selected in a random drawing from among all eligible entries received for that Entry Period. Drawings will be conducted on or about the second business day following the close of each Entry Period by Sponsor or its designated representative, whose decisions are final and binding on all matters relating to this promotion.

Odds of winning depend on the total number of eligible entries received for the applicable Entry Period and are expected to be low.

One win per person for the duration of the promotion. An individual confirmed as a winner in any Entry Period is not eligible to win again and will be excluded from all subsequent drawings, whether or not that individual remains a subscriber.

The potential winner will be notified by email at the address associated with their entry within three (3) business days of the drawing. The potential winner must respond to the notification email within five (5) days of the date it is sent, and may be required to complete, sign, and return an Affidavit of Eligibility, Liability Release, and (where legally permissible) Publicity Release within seven (7) days of receipt.

Failure to respond within the stated period, return of a notification as undeliverable, ineligibility, non-compliance with these Official Rules, or failure to return required documents will result in disqualification and forfeiture of the prize. In the event of disqualification, Sponsor may, at its sole discretion, select an alternate winner from among the remaining eligible entries for that Entry Period or elect not to award the prize for that Entry Period.

Entrants are responsible for maintaining a valid, monitored email address and for ensuring that notification messages are not blocked or filtered.

One (1) prize per Entry Period, consisting of a single electronic gift card redeemable at oura.com in the amount of four hundred fifty U.S. dollars (US$450.00). Approximate Retail Value ("ARV") of each prize: US$450.00. Total ARV of all prizes awarded per Entry Period: US$450.00.

The prize will be delivered by email to the winner's confirmed email address. The prize is the gift card only. Sponsor's sole obligation to the winner is to send the gift card by email, and delivery of the gift card fully discharges Sponsor of all further obligations in connection with the prize.

The winner is solely responsible for all steps, costs, and decisions following receipt of the gift card, including without limitation: creating any account required to redeem it; determining ring size, including obtaining and using any sizing kit and any resulting exchange, reship, or delay; selecting the model, finish, and size; placing the order; any Oura membership, subscription, or recurring fee, whether required to use the ring or optional; shipping, handling, duties, or return costs; any amount by which the purchase price exceeds the gift card value; device, phone, or app compatibility; and all warranty, return, repair, replacement, and customer service matters, which are handled solely by the gift card issuer or retailer.

The winner is also responsible for compliance with any terms, conditions, expiration dates, geographic limitations, or other restrictions imposed by the gift card issuer. Sponsor is not the issuer or seller of the gift card, is not affiliated with the issuer, and makes no representation or warranty regarding the gift card, its redemption, or any product obtained with it. Sponsor will not replace a gift card that is lost, expired, unused, partially used, or unredeemable for any reason, and will not provide cash, credit, or any substitute in its place.

No cash equivalent, transfer, assignment, or substitution of the prize is permitted, except that Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable for any reason.

All federal, state, and local taxes and any other costs or expenses associated with prize acceptance and use are the sole responsibility of the winner. The winner may be required to provide a completed IRS Form W-9 or equivalent tax documentation prior to prize fulfillment.

Prizes are awarded "AS IS" and WITHOUT WARRANTY OF ANY KIND, express or implied, including without limitation any implied warranty of merchantability or fitness for a particular purpose.

Except where prohibited by law, acceptance of a prize constitutes the winner's consent to Sponsor's use of the winner's first name and state of residence for advertising, promotional, and trade purposes in any media, worldwide, in perpetuity, without further notice, compensation, review, or approval. Sponsor will not publish the winner's last name, street address, city, or email address in connection with the promotion.

Any use of a winner's likeness, voice, image, or statements in Sponsor's advertising or promotional materials will be subject to that winner's separate, express consent.

By entering, each entrant releases and holds harmless Sponsor, its parent, subsidiaries, affiliates, advertising and promotional agencies, fulfillment partners, and each of their respective officers, directors, employees, and agents (collectively, "Released Parties") from and against any and all claims, liability, injury, loss, or damage of any kind arising out of or in connection with participation in this promotion or the acceptance, receipt, possession, use, or misuse of any prize.

The Released Parties are not responsible for: (a) any incorrect or inaccurate information, whether caused by entrants, printing errors, or by any equipment or programming associated with this promotion; (b) technical failures of any kind, including malfunctions, interruptions, or disconnections in phone lines, internet service, or network hardware or software; (c) unauthorized human intervention in any part of the entry process or the promotion; (d) technical or human error in the administration of the promotion or the processing of entries; or (e) any injury or damage to persons or property that may be caused, directly or indirectly, in whole or in part, from an entrant's participation in the promotion or receipt or use of any prize.

Sponsor reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the promotion, acting in violation of these Official Rules, or acting in an unsportsmanlike or disruptive manner or with intent to annoy, abuse, threaten, or harass any other person.

CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES TO THE FULLEST EXTENT PERMITTED BY LAW.

Participation in this promotion does not alter the terms of any Physician's Choice subscription. Subscribers may cancel, pause, or modify their subscription at any time in accordance with the applicable subscription terms. Cancellation or lapse of a Hack Your Tummy subscription ends automatic entry for subsequent Entry Periods but does not affect eligibility to enter by the free method described in Section 4(b).

This promotion is a limited-time benefit offered at Sponsor's discretion and is not a term or feature of any subscription. Nothing in these Official Rules creates any ongoing right to entry, to a prize, or to the continuation of the promotion.

Information collected from entrants is subject to Sponsor's Privacy Policy, available at physicianschoice.com/policies/privacy-policy. Entry into this promotion does not by itself subscribe an entrant to any marketing communications; any such consent is obtained separately and may be withdrawn at any time.

This promotion is in no way sponsored, endorsed, administered by, or associated with Meta Platforms, Inc. (Facebook, Instagram), TikTok, Google, YouTube, X, Pinterest, Snap, Amazon, or any other social media, retail, or technology platform. Entrants release each such platform from any and all liability arising from or related to this promotion. Any questions, comments, or complaints regarding the promotion must be directed to Sponsor, not to any platform.

Except where prohibited, each entrant agrees that: (a) any and all disputes, claims, and causes of action arising out of or connected with this promotion, or any prize awarded, shall be resolved individually, without resort to any form of class action; (b) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys' fees; and (c) under no circumstances will any entrant be permitted to obtain awards for, and each entrant hereby waives all rights to claim, punitive, incidental, and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased.

All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of entrants and Sponsor, shall be governed by and construed in accordance with the laws of the State of Colorado, without giving effect to any choice of law or conflict of law rules. Any legal action shall be brought exclusively in the state or federal courts located in Jefferson County or the City and County of Denver, Colorado.

Some jurisdictions do not allow the limitation or exclusion of liability for incidental or consequential damages, so the above may not apply to you.

Sponsor reserves the right, in its sole and absolute discretion, to modify, suspend, shorten, extend, or terminate this promotion, in whole or in part, at any time and for any reason or no reason, without prior notice to entrants and without any liability to entrants. This includes the right to end the promotion or any individual Entry Period, to change the entry mechanics, eligibility criteria, drawing schedule, or prize, and to decline to award a prize for any Entry Period.

Without limiting the foregoing, if for any reason the promotion is not capable of running as planned, including by reason of computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, changes in applicable law, or any other cause beyond Sponsor's reasonable control that corrupts or affects the administration, security, fairness, integrity, or proper conduct of the promotion, Sponsor may cancel, terminate, modify, or suspend the promotion and may select the winner from among eligible entries received prior to the action taken, or elect not to select a winner.

Sponsor's decision to end or modify the promotion does not entitle any entrant to any prize, refund, credit, or other compensation, and does not affect the terms of any subscription.

If any provision of these Official Rules is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall be enforced.
