GLP-1 Constipation: What Helps, What to Take and When to Call
Updated Nov 3, 2026 · 17 sources
GLP-1 constipation comes from the medication slowing digestion, starting with how fast your stomach empties, while the fullness it brings cuts how much you drink.1,2 Fix it in this order: water, fiber built up slowly, daily movement and a set bathroom time. Still stuck? Ask your prescriber which laxative type fits.3 In the weight-management trials of semaglutide 2.4 mg, 24% of adults reported constipation, against 11% on placebo.1
Key points
- Constipation is in every GLP-1 label on this page and runs highest in the weight-management trials: 24% on semaglutide 2.4 mg and up to 17% on tirzepatide, against 11% and 5% on placebo.1,4
- It starts early and lingers: a 2023 expert review puts onset in the first 16 weeks, especially the first 28 days, with a median of 47 days in weight-management patients.2
- Daily fiber is your base: Physician’s Choice Fiber Gummies give 5 g of fiber in 2 gummies (See the label), Easy Mix Fiber 6 g in one spoonful (See the label) and Psyllium Husk Fiber 2 g in 5 capsules (See the label).5,6,7
- If you need a laxative, the 2023 U.S. guideline’s strong recommendations among over-the-counter options go to polyethylene glycol and, short term, bisacodyl or sodium picosulfate.8
Call your prescriber or get urgent care now if you have severe or constant belly pain, vomiting, a swollen belly, no bowel movement and no gas passing, or blood in your stool.1,9,10
GLP-1 constipation relief: what to do and take, in order
Start the day you start the medication: the steps that relieve constipation also help you avoid it, NIDDK says, and a 2023 expert review on GLP-1 side effects agrees.2,3
- Water, all day. Drink plenty of water and other liquids when you take a fiber supplement to help keep things moving. Less appetite can mean less drinking, Cleveland Clinic notes, so sip between meals.11
- Fiber, built up slowly. Adults get 25 to 38 grams of fiber per day, though exact targets depend on age and sex. Raise it gradually over a few weeks, our GLP-1 fiber guide says; it also lists the foods to build meals on.12
- Move daily. Even a few minutes of walking gets your gut moving, Cleveland Clinic says.11
- Set a bathroom time. Try 15 to 45 minutes after breakfast, when eating helps your colon move stool, feet up on a footstool.3
- Still stuck? Ask your prescriber which laxative type fits and whether a lower dose makes sense; an expert review keeps that option open. Never change the medication on your own.2,3
What is the best fiber for GLP-1 constipation?
The one you will take every day, with water. The 2023 U.S. guideline on long-lasting constipation suggests a fiber supplement as a first-line option, with good hydration, and names gas as a common side effect, which is why you ease in.8 Pick the form you will not skip: gummy, powder or capsule. Here is what our labels give you.
Physician's Choice Fiber Gummies, Easy Mix Fiber and Psyllium Husk Fiber are made to support regularity and help with occasional constipation.‡

Timing. The injection labels set no meal timing and never mention fiber; the shots go in at any time of day.1,4,14,15 The semaglutide tablet is the exception. Its label says to take it on an empty stomach in the morning with water (up to 4 ounces), then “wait at least 30 minutes before eating food, drinking beverages or taking other oral medications.”1 Your fiber comes after that. Why does GLP-1 cause constipation?
Three things stack up. Each label on this page says the medication delays gastric emptying, the rate food leaves your stomach.1,4,14,15,16,17 The fullness after a dose leads people to drink less water, a 2023 expert review notes.2 And NIDDK lists too little fiber, liquid and activity, plus changes in how much you eat, among the causes of constipation.9
What is the best laxative for GLP-1 constipation?
No source we read names a laxative for GLP-1 constipation, so the closest guide is the 2023 U.S. constipation guideline for adults. Among over-the-counter options, its strong recommendations go to polyethylene glycol, an osmotic laxative, and to the stimulant laxatives bisacodyl and sodium picosulfate, short term or as rescue.8 Your prescriber picks the type and how long.3
- Polyethylene glycol (osmotic). Response held over 6 months; side effects include abdominal distension, loose stool, gas and nausea.8
- Stimulants. Bisacodyl and sodium picosulfate for daily use up to 4 weeks or as rescue; senna gets a conditional recommendation. Cramping and belly pain are common.8 NIDDK keeps stimulants for severe constipation or when other steps fail.3
- Docusate (stool softener). The guideline reviewed it but makes no recommendation, naming it among common options with little data; the GLP-1 expert review still lists stool softeners as an option.2,8
Does magnesium help GLP-1 constipation?
Magnesium oxide has guideline backing on thin evidence: a conditional recommendation for long-lasting constipation, at very low certainty, from two small trials of 94 people on 1.5 g a day or placebo for 4 weeks. Start lower, the panel says; the effect of citrate and other forms is unknown; avoid it with reduced kidney function.8 Ask your prescriber first.
Which GLP-1 causes less constipation?
The labels do not rank them, and each warns that one drug’s trial rates cannot be compared directly with another’s.1,4,14,15,16,17 The pattern they show: constipation runs higher in weight-management trials than in type 2 diabetes trials. A 2023 expert review sees the same in real-world patients and notes that weight-management doses run higher.2 In tirzepatide’s weight trials, the lowest dose had the highest rate.4
| Medication and dose | Trials the label pools | Constipation on the drug | On placebo |
|---|---|---|---|
| Semaglutide 2.4 mg, weekly injection | Weight management: 3 trials, 2,116 adults on the drug, up to 68 weeks1 | 24% | 11% |
| Semaglutide 7.2 mg, weekly injection | Weight management: 2 trials of 72 weeks, 1,311 adults on 7.2 mg1 | 20% (19% on 2.4 mg in the same trials) | 8% |
| Tirzepatide 5, 10 and 15 mg, weekly injection | Weight management: 2 trials, 2,519 adults on the drug, up to 72 weeks4 | 17%, 14%, 11% | 5% |
| Liraglutide 3 mg, daily injection | Weight management: 5 trials, 3,384 adults on the drug, up to 56 weeks15 | 19.4% | 8.5% |
| Semaglutide 0.5 mg and 1 mg, weekly injection | Type 2 diabetes: 2 trials, 521 patients on the drug16 | 5%, 3.1% | 1.5% |
| Tirzepatide 5, 10 and 15 mg, weekly injection | Type 2 diabetes: 2 trials, 718 patients on the drug17 | 6%, 6%, 7% | 1% |
| Dulaglutide 0.75 mg and 1.5 mg, weekly injection | Type 2 diabetes: pooled trials, 1,670 patients on the drug14 | 3.9%, 3.7% | 0.7% |
How long can you go without pooping on a GLP-1?
No label or guideline we read gives a safe number of days, so do not wait it out. NIDDK counts fewer than three bowel movements a week as a symptom of constipation, along with hard, dry or lumpy stools, and says to see a doctor when self-care does not clear it.9 No bowel movement plus no gas passing belongs to the warning line above.
Does GLP-1 constipation go away?
It tends to outlast the other stomach side effects. A 2023 expert review puts onset in the first 16 weeks, especially the first 28 days, with a median of 47 days in weight-management patients.2 The semaglutide label ties most nausea, vomiting and diarrhea to dose increases but gives no such pattern for constipation.16 Keep the routine going.
Constipation after stopping a GLP-1
No label or review we read gives a timeline. Semaglutide stays in circulation for about 5 to 7 weeks after the last dose, its label says; tirzepatide’s half-life is about 5 to 6 days.1,4 Keep the routine, and tell your prescriber if it does not settle.
Can GLP-1 medications cause a bowel blockage?
It has been reported. Each label on this page lists ileus, intestinal obstruction and severe constipation with fecal impaction among reports received after approval; such reports, the labels note, do not always allow a reliable rate or show the drug caused them.1,4,14,15,16,17 MedlinePlus lists the signs of a blocked bowel: no stool or gas passing, a belly that stays swollen, repeated vomiting, or belly pain that will not go away.10
